This application is essentially a form whereby you electronically sign and authorize SMSU to hold you responsible to understand the policies and procedures on the SMSU Residence Life Guide Community Living, SMSU Student Code of Conduct (Student Handbook), and SMSU Residence Life Housing and Dining Contract.

To authorize and sign the form in question, visit SouthwestNet and log into your account using your StarID and password.\
