Vaccination and Testing Policy

All state employees who are assigned to work at a workplace or otherwise provide agency services outside their home are required to either provide a full COVID-19 vaccination or be tested on a weekly basis based on the State COVID-19 Proof of Vaccination and Testing Policy .

A system wide proof of vaccine and testing management tool has been developed that will allow you to provide your proof of vaccination.  

Instructions for Reporting Vaccination Status

  1. You will access the VaxTrak Proof of Vaccine and Testing tool using your StarID and password.
  2. Once logged in, you will either complete an attestation form and provide proof of vaccination, or if you indicate you have not been vaccinated you will be asked to complete the Testing Consent Form. 
    1. If you complete the testing consent form, you will then be provided instructions on how to schedule your weekly testing time. 
    2. If you have lost your vaccination card or don't have a copy, contact your vaccination provider directly to access your vaccination records.  If you cannot contact your vaccination provider directly, contact your state health department's immunization information system (IIS).  You can find state IIS information on the CDC website.                                         

Weekly Testing Process

You only need to sign the Testing Consent Form once and it will apply during the entire time that mandatory testing under HR/LR Policy #1446 is in effect.  If you wish to rescind your consent after signing the form, you must do so in writing and submit it to Human Resources at

Southwest Minnesota State University has secured individual saliva-based PCR tests from Hennepin Healthcare Clinical Laboratory.  

The University plans to provide a testing clinic in SS143.  The testing clinic will take place from 11 am to 3:45 pm on Monday's and Tuesday's.  Please refer to the Covid19 Testing Calendar for testing dates. 

Testing will occur during work hours and be considered work time.  Once you have tested, you are permitted to come to work while you are waiting for your results.  Some tests can take up to 72 hours to receive results.
