| Proposals of Major Significance | Changes to requirements for majors, minors or certifications. |
| Proposals of Major Significance affecting LEP courses | Proposals designating a course to satisfy or change a course that satisfies an area of the LEP. Complete this form along with the appropriate MnTC form below. |
| Proposals of Less Than Major Significance | New courses, topics courses, course deletions, changes to individual courses including: Name, title, number, grading, credit hours, prerequisites, or description. Include the Course Outline Form when required. |
| Course Outline Form | This form MUST accompany the Less Than Major Significance form in the case of: A new course, changes to the course description, topics course, or changes to an LEP course. |
| Library Resource Checklist | This form MUST be completed with assistance from the librarians for any proposals of new majors, minors, or programs. |
