Campus Security Authority Reporting Form
This form is for use by Campus Security Authorities to make reports directly to University Public Safety.
Campus Security Authorities are designated on campus as part of the federal Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (20 USC § 1092(f)), otherwise known as the Clery Act. Its purpose is to provide the campus community with timely, accurate, and complete information about crime and campus safety, enabling them to make informed decisions to keep themselves safe.
If you are reporting more than one crime, please submit a separate form for each incident.
