Submit a Sexual Violence Complaint

Report sexual assault, dating/domestic violence, stalking, or sexual harassment. You may report to University Public Safety and the University Title IX office.

  • Option to request supportive measures
  • Available to students, staff, faculty, and visitors
  • You choose whether to file a formal complaint

Silent Witness

Submit an anonymous tip about a crime or safety concern. Provide as much detail as possible (who, what, when, where, how).

  • Do not use for emergencies—call 911
  • Photos or file uploads, if available
  • Follow‑up may be limited due to anonymity

Campus Security Authority (CSA) Reporting Form

For designated CSAs to report Clery‑reportable crimes disclosed to them. Include sufficient detail for timely warnings and the Daily Crime Log.

  • For CSAs only; training recommended
  • Do not include personally identifying info if confidentiality is requested
  • Submit within 24 hours of notification when possible

What Happens After You Make a Report?

University Public Safety reviews all reports and may follow up for additional details. Some reports may be shared with other university offices (e.g., Title IX, Student Conduct) as required by law and policy. If you request anonymity, our ability to investigate may be limited.
