Campus Security Authority Reporting Form

This form is for use by Campus Security Authorities to make reports directly to SMSU Public Safety. 

Campus Security Authorities are designated on campus as part of the federal Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (20 USC § 1092(f)), otherwise known as the Clery Act. It's purpose is to provide the campus community with timely, accurate, and complete information about crime and the safety of the campus environment so that they can make informed decisions to keep themselves safe.

If you are reporting more than one crime please submit a separate form for each incident.
